that have gone above and beyond in supporting individuals in recovery.

 

 

Nominate one of your PSCs (Personal Service Coordinator),

case manager, advocate who deserves to be recognized

for helping you become empowered in every way

 

 

Your chance to recognize a PSC that has made a difference
in providing hope and empowerment

 

Nominate your family member, care giver or any

 other significant person who empowered you to handle

emotions, define limits, anticipate relapse, overcome obstacles -

and change your lifestyle to promote recovery. 

 

Nominate a mentor or one of the special case managers you
know who deserves to be recognized for all that they do!

 

Nominate a mental health counselor or psychiatrists who

empowers you to make better decisions

 

Nominate a caring and sensitive staff who helped you
