At this point, client/member/consumer agrees to participate in drug treatment at .......as ordered by the Mental Health Court to help maintain his sobriety and maintain a law-abiding lifestyle.
 
 
As part of this treatment plan, client/member/consumer agrees to attend the following groups and to take his/her medications as prescribed and to provide verification of attendance to the Mental Health Court and to his/her Probation Officer (________________):
