The Effort offers low or no-cost health services at various locations throughout Sacramento. You must meet with an Enrollment and Eligibility staff member prior to scheduling appointments. Sacramento County Primary Health Services
The County Medically Indigent Services Program (CMISP) is a program of "last resort" designed to meet the health care needs of individuals in our community who are not otherwise eligible for health care programs such as Medi-Cal, Medicare, or private health insurance; and who meet the County's "last resort" socioeconomic eligibility standards. CMISP serves adults 19-64 years of age with incomes at approximately 0 - 200% of the Federal Poverty Level (FPL).
