1507 4 21th Street  Sacramento, CA 95814  TEL (916) 441-11 0123 TLCS is a non-profit social rehabilitation that provides housing, living skills training, community support services, entitlement advocacy and outreach services for adults with psychiatric disabilities.

SUPPORTED HOUSING AND ENTITLEMENT PROGRAM (SHEP)


SHEP serves as the central intake site for all TLCS programs with the exception of Project Redirection. Clients can be seen either by appointment or on a walk-in basis. Staff provide emergency assistance and advocacy for homeless mentally ill persons related t housing, food, and Mental Health and Related Services Entitlements including General Assistance, Food Stamps, Social Security, GA and Veterans Benefits Eligibility requirements are different for each individual program offered.

 

SHEP advocates are available for information  M-F from 8am - 4:30pm; closed each Wednesday from 2:30 pm to 4:30 pm. Walk-in Hours are from 8am - 11am.

CAROL'S PLACE

 

Carol's Place is a short term residential facility for homeless adults. It provides 24 hour staffing and supportive services for all residents.

VILLAGE GARDENS


Village Gardens is a residential and supportive services program for people with both developmental and psychiatric disabilities who desire to live independently and need extra support to achieve successful community living. This program is fully staffed 24 hour a day, 7 days a week. Referrals to this program are handled by the Alta California Regional Center - Adult Services Division.

HOUSING OPPORTUNITIES FOR PEOPLE WITH AIDS (HOPWA)


TLCS provides housing and support services to people diagnosed with AIDS or HIV + who are homeless and have a physichiatric disability.

A multi-unit cooperative living programs in an apartment based setting. Staff are on site daily to provide supportive services.

SRO COLLABORATIVE SERVICES PROJECT

This project is designed to support Sacramento residents living at selected downtown SRO hotels through the delivery of a range of quality accessible services. The project is centered in the heart of downtown's business district and is a combined effort of the entire community including local business, residents, faith based organizations, and a number of service providers. This project includes linkages to primary medical and mental health services, life skills training, substance abuse recovery, vocational services, and social activities.
