Mental health services provided in research oriented day treatment and reporting program.

Children and youth in need of mental health services who are referred by the court to the Day Reporting Center.

Probation Officers make recommendation to Juvenile Court.

Mental health services provided to youth directed by the Juvenile Court into this education and substance abuse treatment program provided in collaboration with Sacramento County Office Of Education.

Youth ordered by the Juvenile Drug Court.

Probation Officers make recommendation to the Juvenile Court.

Center

·      Quality Group Home

Provides care and comprehensive assessment of juvenile wards to determine the most appropriate placement for each minor.

Youth who have been

remanded to the Sacramento Assessment Center.

Referral by Department of Probation to the Access Team.

Family Child Community Treatment Program (FCCTP)

·      Quality Group Home

Enhanced mental health outpatient services and interagency service coordination. Services provided at schools, in homes, and the community.

Probation youth with a serious emotional disturbance who are placed locally, or at risk for out-of-home placement.

Referral from Probation to Access Team.

Juvenile Justice Institutions Program

·      Sacramento County

Mental health services to children and youth in Juvenile Justice Institutions.

Youth residing in the Youth Detention Facility and Sacramento County Boys Ranch.

Direct referral by the youth or staff to the JJI Mental Health Team.

County General Funds Realignment

Community Connections

·      Catholic Healthcare West (CHW) Clinic South

Mental health services to children and youth following discharge from

Probation’s Assessment Center, WET Youth Center, or Boy’s Ranch.

Children, youth and families needing mental health

services.

Juvenile Probation refers directly to the Community Connections staff, which completes the Access Team referral.

Multi-Systemic Therapy

·      River Oak

Comprehensive assessment and multi- systemic therapy for probation youth meeting grant criteria.

Probation refers youth meeting MIOCR eligibility with Medi- Cal.

Probation initiates all referrals.
